Important Forms and Information

- Supervisory Committee Form
- Must be submitted prior to or at the same time as your Admission to Candidacy form
- Any Committee Members who are not employed by USF must have a CV attached to the committee form
- Change of Committee Form
- For making a change in the members of your committee or their position on your committee
- Deferment
- To request deferment, send an email to the Graduate Program Director and the Graduate Program Specialist. Include your full nam, the semester you were admitted to graduate studies, and the semester to which you wish to defer.
- Admission can be deferred up to 3 semesters. After that, you must reapply.

- To be submitted via email in place of permit request process
- To be used with courses: EML 7980-Dissertation-Doctoral, EML 7915-Directed Research, EML 6971-Thesis: Master's, EML 6907-Independent Study
- Successful Defense Form
- To be submitted after every Master's/PhD defense
- Please submit directly to Catherine Burton
- Successful Proposal Defense Form
- To be submitted once proposal defense for qualifying examination has been completed
- Please submit to the Graduate Program Specialist
- Defense Assessment Rubric
- Bring a copy for each committee member to your defense
- To be submitted to the ME office only
- Graduation Checklist
- To be submitted to the ME office for advisor approval the semester before you graduate
